Skilled Nursing Facilities NP opportunity in southern MA near Cape Cod and Providence, RI New

Great opportunity for a Nurse Practitioner to work within local Skilled Nursing Facilities (SNF) in southern MA near Providence, RI and Cape Cod. 

• Board Certified Nurse Practitioner (NP)
• Will be delivering care to patients in local skilled nursing facilities (SNF)
• Mon-Fri - 30-40 hours a week - either 4 or 5 days a week
• Start and end times are flexible - ability to step away for family responsibilities, hours and days can be individualized, days can be shifted to weekends if needed
• Coordinated model of care that covers patients from hospital to SNF and then back home
• Goal is to maintain excellent care while in SNF and avoid hospital readmissions
• Nurse Practitioner would work alongside other NPs and each facility’s medical director, determining management plans, and directing the care of SNF patients
• Several years exp as NP is desirable but recent new grads will be considered if they feel they can work independently in these settings
• Portuguese fluency is a “plus” but not a requirement
