Hope Valley, Rhode Island: A respected federally qualified health center invites a Family Nurse Practitioner or Physician Assistant to join its mission-driven primary care team serving patients of all ages in a modern outpatient setting. This is full-spectrum family medicine within a supportive Patient Centered Medical Home model focused on access, continuity, and community-based care. You will join an established clinical team providing services to residents of southwestern Rhode Island and nearby eastern Connecticut.

You can expect a broad mix of pediatrics through seniors in a modern outpatient setting. Providers practice with appropriate autonomy while working alongside physicians, fellow APPs, nurses, and support staff to deliver coordinated care. Administrative time is built into the schedule to support documentation and care coordination.

This practice delivers whole-person care supported by integrated behavioral health, pharmacy, laboratory, and dental resources on-site. Providers benefit from dependable clinical support, experienced colleagues, and a well-organized environment that supports meaningful patient relationships and efficient, evidence-based care.
